Questionnaire

Training for the 15th in-house batch of the School of Medical Transcription, M-Squared Software and Services, Technopark, Thiruvananthapuram began on January 17, 2005.

Our online medical transcription training commenced in June 2004.

Two batches have completed the online training with 100% success rates.
Currently there are 5 batches undergoing online training in medical transcription.



FAQs

Q. What does alert and oriented times 3 mean?

Orientation: Awareness of one's self and one's environment with respect to person, place, time, and event.

It is a measurement of 4 parameters i.e. person, place, time, and event. That means that the person has a functional ability to know and understand who he is, where he is, when it is and what has happened.

Usually only the first 3 parameters are assessed (by means of questions asked by the health professional) and hence the patient is stated to be alert and oriented times 3. In certain instances the patient may be oriented to 2 parameters only and will answer questions vaguely for the rest. In such cases the patient is stated to be alert and oriented times 2.

Q. What is microalbuminuria?

Microalbuminuria is a slight increase in urinary albumin excretion that can be detected using immunoassays, but not using conventional urine protein measurements. This is an early marker for renal (kidney) disease in patients with diabetes mellitus.

Standards

Use of article 'The' with patient:

Start the Subjective part with "The patient" as well as at the beginning of paragraphs.
In other parts of the document, transcribe verbatim.

Examples:

S:	The patient is a 65-year-old Caucasian female...

	SOCIAL HISTORY: The patient smokes 1 pack...

P:	She was prescribed amoxicillin 500 mg q.8 h.  Patient is advised to return to the clinic in 3 weeks' time. 

Followup and follow up

Followup is used as both noun and adjective whereas follow up is used as verb.

Examples:

The patient is here today for followup. (noun)
The patient is here for a followup visit. (adjective)
She will be here for a followup appointment next month. (adjective)
The patient is advised to follow up in 1 week. (verb)

If followup precedes a noun (visit and appointment are nouns in the above examples) it is an adjective.
To differentiate between noun and verb substitute the word 'follow up' with consult and consultation.

The patient is here today for consultation.  Here it is a noun, use followup.
The patient is advised to consult in 1 week.  Here it is verb, use follow up.
